Pathways Is Bringing Care to Longmont


For nearly 50 years, Pathways has walked alongside Northern Colorado families through serious illness, hospice care, grief and some of life’s most tender moments. Now, we’re bringing that same care to Longmont.

Pathways has expanded our service area, which means individuals and families in Longmont can now turn to us for hospice and palliative care, delivered with the compassion, expertise and personal attention that have been at the heart of Pathways for decades.

Our care comes to you, wherever you call home. That may be a private residence, assisted living or skilled nursing community. We also work with local hospitals to help patients and families understand their options and make the transition to hospice care when the time is right.

Choosing a hospice is personal, and quality matters. Pathways is proud to be a 4-Star Medicare-rated hospice based on family caregiver experience as measured through quality scores gathered from patient families. We encourage families to visit Medicare’s Care Compare to see how Pathways performs and learn more about the quality of care we provide.

As we grow in Longmont, our local Community Liaison will be out in the community building relationships with hospitals, physicians, senior living communities and other organizations that care for Longmont residents. More importantly, she’ll be a resource for patients and families, helping people understand their options, ask questions and connect with the right support when they need it.

Our team surrounds patients and families with support that can include nursing and medical care, social work, spiritual care, grief support, volunteers and other services based on what matters most to each individual. Longmont hospice patients also have access to the Pathways Inpatient Care Center in between Loveland and Fort Collins for short-term inpatient hospice and respite care when needs change.

Our connection to Longmont extends even further through our affiliate, The Denver Hospice, which also serves the area. If a patient’s location or needs fall outside what Pathways can provide, we can help connect them to the right care. The goal is simple: help families find the support they need without making them navigate it alone.

Serious illness brings enough uncertainty. Knowing where to turn for help shouldn’t add to it.
