Better Together: Colorado PACE and CVNA Bring Foot Care to the Day Center
Sometimes, meaningful healthcare collaboration starts with a simple question:
How can we make it easier for people to get the care they need?
Recently, Colorado PACE and Colorado Visiting Nurse Association (CVNA) answered that question by teaming up to offer a Medical Foot Care clinic at the Colorado PACE Day Center.
The collaboration brought CVNA’s medical foot care expertise directly to Colorado PACE participants, creating a convenient opportunity to receive preventive foot care without having to travel to a separate clinic.
More than foot care
Healthy feet are an important part of overall health, particularly as we age. CVNA’s Medical Foot Care services can include a comprehensive foot assessment, screening for neuropathy and circulation concerns, blood pressure monitoring, medication review, assessment of chronic conditions and individualized health education.
Regular foot care can also help older adults maintain mobility and independence. By bringing the clinic to the Colorado PACE Day Center, participants could access this important service in a setting where they already receive healthcare, therapy, social support and other services.
Colorado PACE is designed around a comprehensive, team-based approach to care, with services coordinated through its interdisciplinary team and delivered in ways that help participants remain as healthy and independent as possible in their homes and communities.

This collaboration also highlights an important benefit of the Care Synergy network.
