Care Synergy Programs Featured in Forbes Article on the Future of Long-Term Care

How can we help older adults get the support they need to live safely, maintain their independence and remain in the places they call home? A recent Forbes article explores one potential path forward, highlighting approaches that align with the work of Care Synergy and its programs, including the CAPABLE National Center and Colorado PACE.

In “How a Medicare Long-Term Care Benefit Could Save Money, Improve Lives,” published October 23, 2026, journalist Howard Gleckman examines a proposal to expand access to home- and community-based long-term care services through traditional Medicare. The proposed benefit would help eligible older adults access services that support daily living, while potentially reducing avoidable hospitalizations and other costly healthcare needs.

The article highlights CAPABLE as a potential component of the proposed benefit and discusses how adult day services could incorporate elements of the Program of All-Inclusive Care for the Elderly (PACE).

Together, these approaches illustrate the value of looking beyond medical treatment alone to address the broader needs of older adults.

Helping Older Adults Live Safely at Home

For many older adults, remaining at home means maintaining familiar routines, staying connected to their communities and continuing to make decisions about their daily lives. But changes in mobility, health or the ability to manage everyday activities can create challenges that threaten that independence.

The CAPABLE National Center advances a person-directed approach designed to help older adults address those challenges. CAPABLE, which stands for Community Aging in Place – Advancing Better Living for Elders, brings together occupational therapists, registered nurses and trained home-care workers to help participants identify personal goals, improve home safety and develop practical strategies for daily living.

By considering both an individual's needs and their home environment, CAPABLE helps address barriers that can increase fall risk or make everyday activities more difficult. The goal is to help older adults remain safe and independent in the homes they love.

Care Synergy and the CAPABLE National Center were also recognized by the Elevance Health Foundation through its inaugural Patient Safety Prize for their work in fall prevention, underscoring the importance of practical, community-informed approaches to improving safety.

Bringing Healthcare and Supportive Services Together

While CAPABLE focuses on helping older adults overcome specific challenges at home, PACE takes a comprehensive approach to coordinating healthcare and supportive services.

PACE, or the Program of All-Inclusive Care for the Elderly, serves eligible older adults who meet nursing-home-level-of-care requirements but can continue living safely in the community with PACE support.

Through Colorado PACE, participants receive coordinated care from an interdisciplinary team that considers their medical, physical and social needs.

Depending on individual care plans, services may include medical care, prescription medications, physical and occupational therapy, transportation, personal care, meals and opportunities for social connection.

By bringing these services together, Colorado PACE helps participants and their families navigate complex care needs while supporting health, safety and quality of life.

The Forbes article discusses how a proposed Medicare benefit could draw on elements of PACE, including adult day services, as part of a broader approach to supporting older adults who need assistance with daily activities.

A Shared Commitment to Independence and Well-Being

Although CAPABLE and PACE operate differently, both reflect an important principle: Supporting older adults means understanding the whole person and providing practical services that respond to their individual needs.

CAPABLE helps participants identify and address barriers to safe, independent living at home. PACE coordinates medical care and supportive services to help eligible older adults remain in their communities. Both approaches recognize that health and independence are shaped by more than medical care alone.

The Forbes article explores how expanding access to home- and community-based services could improve quality of life while potentially reducing healthcare costs. The Medicare benefit discussed is a proposal, not a currently available benefit, but the conversation highlights important opportunities to strengthen support for older adults and their families.

At Care Synergy, we are proud to support approaches that help people receive meaningful, coordinated care while maintaining as much independence as possible. Through programs such as the CAPABLE National Center and Colorado PACE, we continue to advance practical, person-centered solutions that make a difference in people's lives.
